Please note:  Current students (22-23) from other Catholic schools inour diocese are welcome to join our summer program.  Please be sure to tell us which school your student has attended  below.  A form requesting addtional info will be emailed to you.
 

Please indicate the drop off and pick up times you will need for your child in order to accommodate your work or family schedule. Please write "varies" if necessary!

Please indicate your child's attendance for each week below.  If you choose full week, please do not mark off individual days.

The Summer Program begins the day after the last day of school year. 

Upon registration (above) you will be billed a non-refundable fee of $200 through FACTS, payable by June 9th to confirm your child's registration. This fee will be applied to the final days/weeks for which your student has registered above.

Should you need to change your child's registration before June 1, please submit a new registration form here. The most recently dated registration will be accepted as the correct registration for your child.

Once the Summer Program begins, full payment of reserved days/weeks is required and non-refundable.  Prepayment for the week is due on Monday of each week your child is attending. A minimum of two weeks written/email notice must be given if you choose to cancel reserved days/weeks in order for charges to be waived.

By submitting this form you are acknowledging that you are requesting the days above to be reserved for your child and that you are financially responsible for payment as described here and on the Summer Program page of the ASCS website.
